Project Extension for Community Healthcare Outcomes® (ECHO) is a well-established, innovative, and effective intervention to strengthen primary care infrastructure for medically underserved patients. Through videoconferencing and case-based learning, Project ECHO CCM provides nurses with expert case consultation, education, and support, enabling them to more effectively manage complex patient cases.
